- move plugin and mCdm creation in SetUp() test fixture
- replace StrictMock<MockCD<> with NiceMock; otherwise,
"uninteresting mock for isOpenSession" will return fail by default
- replace .WillOnce() for isOpenSession() with .WillRepeatedly
